Duncanville ISD staff members shall follow these procedures regarding personal
identification when enrolling students at the campus.
Returning Students (except students enrolling under an Affidavit of
Residence - see “Yellow Form” section)
Provide a current original utility bill, or other proof of residence such as a
lease contract.
Students New to the District
The parent or guardian must present an acceptable photo ID (see list below). The
ID must have the parent/guardian’s photo, name, and current address. The address
on the ID must match the presented original utility bill or other proof of
residence. Without all of the above, a new student cannot begin the enrollment
process during pre-registration.
All Students Enrolling Under an Affidavit of Residence (Yellow Form)
Both the parent/guardian and district resident (host family) with whom the
parent and student are residing must accompany the student at enrollment. Both
the parent/guardian and host family must present an acceptable photo ID (see
list below) to enroll the student. The photo ID of the parent/guardian must have
the parent’s photo and name, and the address on the ID must also match the host
family’s proof of residence. The host family with whom a parent/guardian and
student are residing with must present a photo ID that reflects their current
address. The current address on the host family’s ID must match the current
original utility bill or other proof of residency. All of this documentation is
required, in addition to the notarized Affidavit of Residence form, to enroll
the student.
Acceptable Photo ID must be current and valid, and may include any of the
following:
• State issued driver’s license
• Texas ID
• Passport with current address
• Military or government-issued ID
• Consular Card
Non-Acceptable Photo ID:
• A membership card such as Sam’s Club or other similar cards
• A debit/credit card with a photo, etc.
To enroll a student in the Duncanville Independent School District, the
parent/legal guardian under a court order, attorney-in-fact, and/or district
host family must accompany the student. The district requires the following
documents to complete enrollment, and cannot begin the enrollment process during
pre-registration without all of the documents listed below:
• Proof of Residence: original current utility bill, not a telephone
bill.
• Valid Parent/Guardian Identification: photo ID with an address that
matches the utility bill.
• Proof of Student Identification: original birth certificate, valid
passport with visa, or other legal document that establishes identity.
• Copy of student’s social security card
• Copy of student’s immunization record
• Student’s last report card/transcript or withdrawal paperwork from last
school attended
